MEDICAL
EXAMINATIONS FOR THE ADJUSTMENT OF STATUS APPLICATION
Every applicant for adjustment of status must undergo a complete medical
examination by a doctor designated by the U.S. Citizenship and Immigration
Service (USCIS). To obtain a listing of doctors, approved by the USCIS, in your area,
call 1-800-375-5283.
In accordance with the 1996 Act, all applicants for adjustment of status
must prove that they have been vaccinated against vaccine preventable
diseases. This requirement can be waived under either of the following two
conditions: (1) a civil surgeon or similar official certifies that such
vaccination would not be medically appropriate; or (2) the requirement of
such vaccination is contrary to an applicant's religious beliefs or moral
convictions.
To comply with the vaccination requirement, you must present a Supplement
for Form I-693 in which a civil surgeon certifies one of the following:
1. You are in full compliance with the vaccination requirement;
2. You lack certain vaccines but are eligible for a "not medically
appropriate" waiver; or
3. You have religious or moral reasons for refusing the vaccine.
During the medical examination, you must present proof of vaccination
against mumps, measles, rubella, polio, tetanus and diphtheria toxoids,
pertussis, influenza type B, hepatitis B, varicella (chickenpox),
haemophilus influenza type B, and pneumococcal vaccines.
